Please note, we will be unable to accept retainer agreements after 5:00 pm, March 1, 2022. If you fill one out on-line after that time, we will be unable to countersign it and will be unable to file a grievance for you. Any requests for a retainer agreement by mail or email also will be unable to be fulfilled. Thank you for your understanding. If you still need a grievance filed after March 1, 2022, you may be able to find another provider that is still accepting applications.
